Indemnity Max Plan - Voluntary & Contributory Features

- Covers preventive care at 100% (contracted providers)
- Includes child orthodontic insured benefit (for groups of 10 or more enrolled employees)
- Includes adult and child orthodontic discount (contracted providers)

- Member maximums increase yearly
- The initial plan maximum can be customized
- For every consecutive year on the plan each member will receive increased maximums as follows**
- Year 2 - $100
- Year 3 - $200
- Year 4 - $300
- Year 5 - $400

- Both plans include specialist discounts. Contracted fees apply when members visit a contracted provider
(at subscribers effective date to a maximum of $2,000)

Minimum Enrolled Employees: 2
Minimum Contribution: None
Minimum Participation: 2-5 on census - 100%participation
6 or more on census - Minimum of 5 enrolled
Contributory Plan Requirements
Minimum Enrolled Employees: 6
Minimum Contribution: 50% of single premium
Minimum Participation: 75% of eligible employees

**Different starting maximums are available by quote, yearly increments cannot be changed. All plan maximums cap at $2,000.
